Compressed Gas Cylinder Return Form

This return form can only be used for gases bought under the old contract (before March 1, 2000). Any gas bought under the new system must be returned through Airgas. Please use this form to return any compressed gas cylinders no longer needed. If you have any questions regarding this form, please call 453-7180 or e-mail aruffing@cehs.siu.edu.

Pickups occur on a monthly basis (or more frequently as needed). Please have someone available to admit transporters to the tank rooms.

Requirements for Tank Removal

Compressed gas cylinders must be clearly marked “Empty” or “Remove this Cylinder”
Remove all regulators, tubing and chains/straps from tanks.
Tanks must have the cap firmly screwed on. We will not transport tanks without caps. If you cannot find the cap for your tank, you must purchase one from CEHS. Each cap costs $5.00.

color="#FF0000">*The tag number is required to have the cylinder returned to CEHS. This form will be disregarded if the tag number is not included. The tag number may be found on a manila tag wired to the cylinder or a sticker placed on the tank. If no tag number is present, please send the serial number to aruffing@cehs.siu.edu or call 453-7180 to receive a tag number.
