This form is to be used for reporting problems
with your ventilation hood. It is important ALL information on the
form is completed. A representative from the Center for
Environmental Health and Safety (CEHS) will contact you to discuss
the hood deficiency if necessary prior to evaluating the hood. If
necessary, CEHS will complete a Physical Plant Work Order to have
the hood fixed. Please e-mail
