Incident/Hazard Reports

The Center for Environmental Health and Safety (CEHS) should be notified within 24 hours when there is an incident/hazard on campus that has the potential to cause property damage and/or injury or illness to employees, students or visitors. These incidents/hazards could be either facility conditions or personnel operations that are unsafe and therefore need correction. Incidents/hazards can be reported by submitting a completed Report of Incident/Hazard form to CEHS (see forms below).

Life threatening or otherwise serious hazards should be reported immediately to CEHS at 453-7180 or after hours to Facilities Operations Center at 453-3621.
