Planned Purchase Order Requisitions for Radioactive Materials

Radiological Laboratory Supervisors that have been granted permission by the Radiation Safety Officer to utilize planned purchase orders for radioactive material orders shall complete the following form for each order placed prior to its delivery to the SIU Carbondale Campus (note: Planned Purchase Order has replaced the term Blanket Requisition in Purchasing). Please remember to have all packages delivered to:

We recommend the expected day of deliveries to be a Tuesday, Wednesday, or Thursday. We have experienced a few problems with deliveries on Mondays and Fridays. Occasionally shipments due on a Monday may be shipped on a Friday, leaving the package to sit out for a weekend in delivery agency’s warehouse. Friday deliveries occasionally have not been delivered on time, and also sat in a warehouse until the following Monday. This presents a problem for packages that need to be refrigerated.
